I have a 2004 ES330 in the same way as a hestitation problem. It is fine from a full end but subsequently I go something like turns on onto highway ramps, next I upset the accellarator (sp?) it hesitates for 2 seconds since reving up. I ve taken it to a mechanic and he says it is normal. I have an 2003 Avalon like the similar engine and it jumps off the start and has no hesitation. I did re-check them tho. The master tech did extensive tests upon the let breathe flow meter and said it was good. as soon as I unplug it and control the motor, the car runs rasping and at 850ish. I can unaided guess that my new rebuilt ecu has an issue, or the tps needs to be re-adjusted, or someway my maf has died in the midst of all this. I don't think the tps has that much direct in the swap of rpm though? I think it solitary adjusts it in this area 1k..
have you looked into throttle body coking and/or combustion chamber/valve carbon buildup? are you up to date upon all maintenance; spark plugs fuel filter pcv vlave etc? have you performed a visual inspection and listened to engine as it warms happening and is warmed up? oftentimes in engine play in many factors may cause the similar symptom(s)
